Optimal Care, Inc. is a Medicare-certified skilled home health care service provider that has been nurse-owned and operated since 2006, and opened our hospice division in 2021. We serve homebound adult patients in the Wayne, Oakland, Macomb and southeast Livingston counties. Our team consists of postgraduate professional caregivers who are trained in geriatrics and are experts in developing comprehensive patient care plans.
