Job Summary:

In this role, you will engage individuals experiencing housing instability, assist with accessing housing and community resources, and collaborate with local Continuums of Care, landlords, and service providers. You will serve as a subject matter expert on housing resources while supporting crisis resolution, benefit access, and long‑term housing success.

Key Responsibilities:
· Engage individuals experiencing homelessness or housing instability using Housing First, harm‑reduction, and low‑barrier approaches
· Participate in Coordinated Entry and Case Conferencing meetings within the local Continuum of Care
· Conduct outreach to individuals not currently engaged in services and connect them to appropriate supports
· Assist individuals with accessing behavioral health, medical, employment, and other essential services
· Build and maintain strong relationships with local landlords and provide education on available programs
· Connect referrals from Continuum of Care partners to appropriate housing and support resources
· Utilize SOAR training to assist individuals with applying for benefits
· Support individuals in securing identification and documentation needed for housing or employment
· Assist individuals in transitioning to stable, permanent housing and evaluate readiness for independent living
· Reengage individuals who have disengaged from services or are at risk of returning to homelessness
· Identify and connect individuals with natural supports and community resources to sustain housing stability
· Maintain accurate and timely data collection and entry as required by regulatory standards
· Serve as a resource to team members, clients, and leadership regarding housing and community services
· Participate in required trainings, statewide housing liaison calls, and program initiatives
· Perform other duties as assigned
Education, Experience, and/or Credential Qualifications:
· Bachelor’s degree in Social Work, Psychology, Sociology, or related field or relevant case management experience in housing
· Associate of Applied Science in Behavioral Health Support from an approved institution or eligible to provide community support services
· Must meet Community Support qualifications under Missouri guidelines, including any combination of education and qualifying experience
· Qualifying experience must include service delivery to individuals with mental health, substance use, or developmental disabilities
Additional Qualifications:
· Must have a valid driver’s license with an acceptable driving record and current auto insurance
· Must complete SOAR training and Housing Liaison training provided by the Department of Mental Health
· Must complete HUD Quality Standards training/quiz
· Ability to work effectively with diverse populations and maintain confidentiality
· Strong communication, relationship‑building, and organizational skills
· Ability to multi‑task and respond effectively in crisis situations
· Proficiency in Microsoft Office and data management systems
Physical Requirements
· Ability to exert up to 10 pounds of force occasionally
· Ability to perform repetitive hand, finger, and arm movements for typing and documentation
· Ability to sit for extended periods with occasional walking or standing
· Ability to meet the physical demands of a sedentary work environment
