Compass Health Center is seeking a Mental Health Resource and Discharge Planning Therapist for our Partial Hospital and Intensive Outpatient Programs at our Chicago, IL office.
We are seeking an experienced clinician with a passion for delivering evidence-based therapy to Adults and Young Adults with anxiety and mood disorders. As the resource and discharge planning therapist, you will be responsible for facilitating aftercare planning with patients and families as well as providing general skills groups as needed.

This position is embedded within the Chicago Over 18 programs and will report to the Sr. Director of Adult and Young Adult programming.
Your responsibilities include but no limited to:
- Supports patients/loved ones in aftercare planning including therapy, psychiatry, and group referrals.
- Manages referral lists and develops relationships with referral sources
- Problem solves difficult discharge issues including limited insurance pools and specific discharge recommendations/requests
- Communicates with treatment teams about individual patients’ needs
- Facilitates communication among important stakeholders supporting patients inside and outside the organization
- Develops referral materials- refining, improving, and developing to respond to growth and change within the organization
- Facilitates general groups as needed as well as any groups related to discharge planning or support in PHP/IOP or aftercare programs
- Gathers feedback and collects data about discharge processes; responds to this feedback by making improvements and necessary changes
- Works on initiatives with senior leadership related to improving the discharge process such as creating an aftercare group
Qualifications (necessary skills and experience required):
- Master’s degree in Counseling, Psychology, Social Work, Marriage and Family therapy or related degree
- Case management experience
- Excellent time management
- Clinical experience conducting intakes, assessments, individual, family and/or group experience
- Knowledge and experience of group dynamics utilizing Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), as well as other empirically supported treatments.
- Ability to juggle multiple demands simultaneously in a fast paced and changing environment
