Location: Across NCN Health
EFT/Hours: 0.2 EFT (16 Hours Per Fortnight)
Status: Part Time Fixed Term until June 30, 2027
Hourly Rate: Dependent upon qualification and skill level and 12 % supperannuation
About the Role
NCN Health has an exciting opportunity for a Clinical Coordinator –Murray PHN Chronic Disease Exercise and Education Program to join our Allied Health and Chronic Disease team.
The Clinical Coordinator is responsible for the implementation and coordination of the newly funded Murray PHN Chronic Disease Exercise and Education Program. This program is funded for 12 months and is in partnership with Yarrawonga Health. The program will deliver direct clinical services through a structured exercise and education group program for people living with chronic conditions - cardiovascular (CVD), chronic obstructive pulmonary disease (COPD), and diabetes who reside within the Moira Shire. The program will be delivered at each of the 3 campuses both in-person, virtually and will also include some outreach locations.
The Clinical Coordinator will work closely with all project partners and team members to ensure that all elements of the program are maintained, and all reporting and budget requirements set by Murray PHN are met.
The Clinical Coordinator will be key in the engagement with internal and external stakeholders and collaborate with community groups, enabling connections with potential participants for the program.
