The Team:
The Discharge Planning Team are committed to delivering the best outcomes to patients across the North-West region. Demonstrating experience, understanding and expertise in the clinical management of patients with complex discharge requirements.
The team works in partnership with relevant stakeholders such as Nurse Unit Managers, Nurse Manager Patient Flow, Allied Health and Community Liaison Nurse, to co-ordinate the discharge processes for patients within the hospital to ensure timely access to services and or care.

As a member of the multidisciplinary team within Nursing Services, the Clinical Nurse Consultant - Discharge Planning provides clinical leadership and direction in the planning, implementing and evaluation of change management processes to ensure efficient and effective discharge processes across the Mersey Community Hospital (MCH). Details of appointment:
Fixed term, part time, day worker position, working up to 48 hours per fortnight, commencing from 19 July 2026 until 30 June 2028.
*notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.
Salary: $118,297 to $125,543 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
Access to the Grade 4 salary range $102,439- $109,175 is subject to successful application for progression to Grade 4
