JOB DESCRIPTION:
The Nursing Supervisor – Health Centers will supervise clinical nursing services at PHMC Health’s community health centers. This includes clinical oversight for the RNs, LPNs, and MA’s, conducting periodic audits, monitoring and implementing improvements in clinical operations and quality metrics, managing the corrective action planning process, overseeing regulatory compliance, facilitating policy development, and coordinating outcome reporting and partnerships/communications with internal and external partners to ensure adherence to the overall health center mission. The Nursing Supervisor – Health Centers will report to the Director of Nursing and serve on PHMC Health’s nursing leadership team, along with the Nursing Supervisors for Shelter Nursing and Medical Respite.

Supervision and Management:
• Participate in the management of the team in coordination with PHMC Health leadership, in particular the Medical Director and other nursing supervisors.
• Provide professional guidance and verbal direction to RNs, LPNs, and MA’s as directed by the Medical
Director.
• Develop, provide, and/or coordinate nursing education and professional development activities.
• Assist in the recruitment and orientation of new clinic RNs, LPNs, and MAs.
• Assist in the coordination of nursing student rotations.
• Engage with PHMC and other community partners in nurse-led initiatives.
• Develop and oversee the implementation of competencies to ensure continued improvement in clinical skills
Strategy Development.
• Identify and analyze significant opportunities for improvement in care and service.
• Through engagement and collaboration, work with leadership and staff to make quality and its associated
processes intrinsic to everyday practices.
• Annually develop and implement the health centers’ total quality management strategy with policies, programs, and initiatives.
• Capable of resolving escalated issues arising from operations and requiring coordination with other departments.
Clinical Quality:
• Design and implement QI activities carried out on an ongoing basis to promote efforts supporting the identification and correction of quality-of-care issues.
• Evaluate current standards of care, establish acceptable standards of care throughout all product lines, and promote the most effective use of resources to reach acceptable and high standards.
• Ensure adherence to established accreditation/HRSA, DHS, Joint Commission, and other federal/state and local requirements.
• Foster improvement strategies, along with systematic tracking, data-driven decision making, to progress towards established benchmarks or goals.
• Align the goals and objectives to improve the quality of clinical care and services provided by the care delivery system in all settings, especially as it pertains to the unique needs of the population.
• Provide technical assistance to managers to build quality into ongoing work processes.
• Foster continuity and coordination of care between primary care practitioners, behavioral health providers, social service staff, and other specialists and nursing staff.
• Monitor, identify, and investigate potential quality issues, conduct root cause analysis, and implement corrective action.
• Assist in the preparation for and participate in regulatory audits,s including HRSA, DHS, Joint Commission, VFC, and Title X.
• Assist in supply management for clinic-distributed medications, vaccines, and clinic supplies.
• Develop health services measures of QI, in partnership with the CQI Committee.
• Partner with Information Technology (I.T.) to develop the dashboards for performance indicators
• Conducts routine site monitoring visits.
• Attends UDS measures meetings as requested.
• Assists with troubleshooting Electronic Medical Record issues.
• Assume new responsibilities as new needs are identified.
Infection Control/Emergency Preparedness:
• Provides coordination and monitoring services for reports of suspected or confirmed communicable diseases that can be transmitted through routine primary care.
• Monitor health centers for consistent hand washing, care of the environment, use of safety syringes and sharps disposal, and use of personal protective equipment.
• Manage or participate, as needed, in communicable disease outbreaks, emergency preparedness/disaster response, and other health initiatives.
• Train community and academic health care professionals to provide comprehensive, culturally competent, continuous care to the health center patient population.
• Provides monthly tracking and record keeping on-site for the environment of care report monthly. Discuss any deficiencies with PHMC Health leadership and monitor remediation of deficiencies.
Clinical:
• Provide on-call /on-site consultation for health center patients, adjunct to community health nursing services as needed (e.g, no nurse available).
• Provides resources for primary care to health center staff, as indicated.
• Document all services promptly.
• Participate in advocacy and coordination committees addressing issues related to community health.
• Collaborates with DPH, OHS, and shelters for timely resolution of infection control measures. Reviews, records,s and submits monthly CLIA lab reports.
• Reviews, records, and submits monthly AACO reporting measures.
• Oversees seasonal influenza vaccine process in collaboration with site nurse(s).
