RATE OF PAY/SALARY: $30.22 per hour
WORK LOCATION(S): Middletown, NY
STATUS: Full-time
Schedule: Monday-Friday 9AM-5PM

General Purpose:
Provide comprehensive medical case management services to Cornerstone Family Healthcare patients living with HIV (PLWH).
Description of Duties:
- Provide comprehensive case management services to PLWH.
- Provides a client-centered service that links patients with health care, psychological and social services in a manner that ensures timely, coordinated access to medically appropriate levels of care and support services and continuity of care.
- Develops linkages and networks with other groups and agencies offering services to PLWH to help promote cooperation and reduce duplication of services among agencies.
- Establishes peer support groups and focus groups for our patients, and PLWH referred from other agencies.
- Provide crisis intervention to patients.
- Provides follow-up and outreach for those patients not engaged in care.
- Provide documentation to reflect patient interaction, outreach attempts service plan updates, and biannual psychosocial assessments.
- Performs updates and maintenance of patient charts.
- Provides case management input and psychosocial information concerning patients during medical conferences.
- Enter case management data into EMR and AIRS databases.
- Attends conferences and various training sessions, as needed.
- Refers patients to programs, support groups and medical treatment, as needed.
- Develop relationships and represent Cornerstone Family Healthcare in community organizations.
- Attend and participate in monthly department, All Staff and other required meetings.
- Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family Healthcare Policy and Procedure Manual and Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other duties as assigned.
Requirements
- Must be Bilingual (English/Spanish)
- Bachelor of Science/Art in Social Work, Psychology, Human Services or related field.
- Highly organized with excellent oral and written communication skills.
- Ability to maintain a nonjudgmental disposition and communicate with a diverse population.
