Summary:
The Outreach Family Support Worker plays a key role in delivering high-quality early help and family support services to vulnerable children, young people, and families across the community. Working within Children's Social Care, the post holder provides targeted outreach and home-based interventions using a whole-family approach to improve outcomes, reduce risk, and prevent escalation to statutory services. The role involves supporting children and families with complex needs, undertaking assessments, coordinating multi-agency plans, promoting safeguarding, and helping families access universal and specialist services. The post holder will work closely with social workers, schools, healthcare professionals, and partner agencies to ensure children are safe, supported, and able to achieve positive outcomes.
