Outreach Family Support Worker

We are working closely with local authority in Liverpool to assist with the appointment of a Outreach Family Support Worker, on a 3-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £18.40- £24.26 per hour

Summary:

The Outreach Family Support Worker plays a key role in delivering high-quality early help and family support services to vulnerable children, young people, and families across the community. Working within Children's Social Care, the post holder provides targeted outreach and home-based interventions using a whole-family approach to improve outcomes, reduce risk, and prevent escalation to statutory services. The role involves supporting children and families with complex needs, undertaking assessments, coordinating multi-agency plans, promoting safeguarding, and helping families access universal and specialist services. The post holder will work closely with social workers, schools, healthcare professionals, and partner agencies to ensure children are safe, supported, and able to achieve positive outcomes.

Responsibilities:

• Manage and support complex family cases referred through Early Help or stepped down from Children's Social Care.
• Act as Lead Professional and coordinate Team Around the Family (TAF) meetings.
• Monitor risks, identify safeguarding concerns, and take appropriate action in line with statutory safeguarding procedures.
• Contribute to service development, continuous improvement initiatives, and best practice sharing within Children's Services.
• Ensure compliance with council policies, health and safety regulations, confidentiality standards, and professional practice requirements.

Essentials:

• GCSEs (or equivalent) including English and Mathematics.
• NVQ Level 3 in Health & Social Care, Childcare, Early Years, Youth Work, Family Support, or related discipline OR equivalent relevant experience.
• Understanding of safeguarding legislation, child protection procedures, and early help frameworks.
• Knowledge of child development, family support principles, and multi-agency working practices.
• Experience working with children, young people, and families in a social care, education, youth work, or community support environment.
• Experience supporting vulnerable families with complex social, emotional, or behavioural needs.
• Experience undertaking assessments, care planning, or delivering targeted interventions.
