San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
Job Summary: Under the direction and supervision of the Clinical Services Manager, the Home Care Coordinator performs in-home safety evaluations by using established clinical and environmental parameters to identify potential hazards, barriers to care delivery, and functional limitations. The Home Care Coordinator supports the safe and effective delivery of in-home services by coordinating assessments, monitoring care plan compliance, and conducting home safety evaluations. Working closely with the Interdisciplinary Team, this role ensures participant environments are appropriate for care delivery, services are initiated in compliance with regulatory requirements, and home care needs are communicated clearly across disciplines.
Duties and Responsibilities: • Complete the initial, semi-annual, and change-of-condition in-home safety assessments in compliance with CMS guidelines. Document all home safety assessments accurately and within required compliance timeframes. • Maintain timely and accurate documentation to support compliance, care coordination, and continuity of services. Complete in-home service plans and attend plan of care meetings. • Monitor and update participant demographic information within the care program to ensure accuracy of addresses and contact details. • Schedule and complete home safety surveys for participants with address changes prior to initiation of in-home services, or within 30 days for all other participants, to ensure environments are safe for service implementation. • Identify durable medical equipment needs and coordinate approval through the Rehabilitation Department prior to ordering. Submit referrals to light housekeeping and laundry vendors when indicated. • Evaluate the appropriateness of light housekeeping and laundry services through internal staff or external vendors and communicate recommendations to the IHS Manager and Clinical Services Manager. • Provide detailed visit-specific information to home care staff, including parking instructions, pet considerations, and directions or methods to locate the participant’s residence. • Review the Welcome to PACE Home Care Services document with participants and obtain required signatures. • Shall attend in-service education program, including orientation and skill training and continuing education. • Shall receive in-service training in first aid and in cardiopulmonary resuscitation within the first six months of employment. • Follows all St. Paul’s PACE Policies and Procedures and OSHA safety guidelines. • Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families. • Practices Universal precautions and follows appropriate Infection Control procedures. • Maintains safe working environment. Follows St. Paul’s PACE Safety Policies and Procedures. • Participates in and supports Quality Improvement initiatives. • Performs other duties as required or requested
Qualification and Requirements:
• Education - California Certified Nursing Assistant (CNA) license. CPR and First Aid certification required. • Experience – A minimum of one year’s experience in working with the frail or elderly population is required. Experience in home care, long-term care, or community-based services preferred.
