We are looking for a nurse practitioner with an autonomous license to join our team full-time at Conviva Primary Care. This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.

Main Responsibilities
- You will deliver comprehensive outpatient care to adult and senior patients.
- You will maintain accurate medical records and documentation.
- You will diagnose and manage moderately to complex medical conditions.
- You will coordinate referrals and collaborate with specialists.
- You will participate in clinical quality improvement projects.
- You will work with interdisciplinary teams to ensure holistic care.
- You will contribute to projects and innovations in care delivery.
Use your skills to make an impact
Required Qualifications
- Three years of experience in value-based care or managing adult patients with chronic medical conditions
- Masters' degree as a nurse practitioner from an accredited medical school.
- Active, autonomous nurse practitioner medical license in the state of practice, or to obtain
- Board Certification
- If selected for this role, you will be required to be screened for TB
Must be willing to participate in limited on-call coverage by phone

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$136,200 - $187,400 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
