A Brief Overview
The Advanced Practice Provider is responsible for providing quality care for patients and increasing patient access and satisfaction. Standard tasks to be performed by a NP/PA as specified by the Ohio State Board of Medicine and UH entity specific Medical Bylaws:
What you will do
In this role you will work 40 hours/week in a hybrid setting. Majority is virtual, however, there is the occasional need to be in office at the Management Services Center. You will conduct appointments for patients in the Healthy at Home Virtual Clinic program, where majority of the patients were recently discharged from UH. You will follow the patients for the duration of the 30-day program, conducting each weekly visit like a PCP office visit. You will review and order medications, labs, etc. Appointments are pre-scheduled, just as in a traditional clinic, however, there are times when a patient may call in and need a same day appointment and will be added to the schedule.

Education Qualifications
· (MSN) Master's Degree in Nursing, or
· Master's Degree from an accredited PA educational program
Experience Qualifications
· 1+ years as a NP or PA Preferred
Skills and Abilities
Experience managing patients with complex conditions (e.g., heart failure, diabetes, COPD)
Familiarity with diagnostic testing interpretation
Experience in acute care, inpatient, ICU, or step-down settings
Experience with care coordination across the continuum (inpatient, outpatient, and procedural areas)
Proficiency in electronic medical records (e.g., Epic or similar systems)
Strong interdisciplinary collaboration skills with physicians, nurses, and ancillary teams
Demonstrated ability to work in a high-volume, independent, fast-paced clinical environment
Participation in quality improvement, research, or evidence-based practice initiatives
Excellent communication skills with patients, families, and care teams
