We are now looking for an Integrated Care Coordinator (ICC) to join our team in Ellerslie. This is an opportunity to play a meaningful role supporting patients and whānau, particularly Māori, Pacific, and high-needs communities, to access the care and support they need to live healthier lives.

About the Role
As an Integrated Care Coordinator, you will work closely with clinical and operational teams to support patients through preventative health programmes, care coordination, recalls, and health education initiatives.
This is a varied and rewarding role combining patient engagement, health promotion, administration, and care coordination. You’ll help improve health outcomes by supporting patients to stay connected with important healthcare services and screening programmes.
You’ll be involved in initiatives including:
Immunisation
Diabetes and long-term condition support
Cervical, breast, and bowel screening
Cardiovascular risk assessments
Smoking cessation and alcohol harm reduction programmes
Community outreach and patient engagement
This role is based at our Head Office in Ellerslie, Auckland, working Monday to Friday, 8:30am – 5:00pm.
What You’ll Be Doing
Supporting patients and whānau in accessing healthcare services and preventative programmes
Coordinating patient recalls, bookings, follow-ups, and outreach activities
Communicating with patients via phone, SMS, and email
Promoting healthy lifestyle choices and preventative healthcare
Supporting patients with long-term conditions and screening programmes
Maintaining accurate patient records and administrative documentation
Working collaboratively with clinic teams and wider support services
Contributing to initiatives that improve patient engagement and community health outcomes
What We’re Looking For
Qualification in health promotion, public health, healthcare, or a related field
Understanding of the New Zealand healthcare system and health equity principles
Strong communication and relationship-building skills
Confidence working with diverse communities and high-needs populations
Organised, proactive, and compassionate approach to patient care
Ability to work collaboratively within a fast-paced team environment
Knowledge of Medtech Evolution or another patient management system (PMS) is an advantage
Ability to speak a second language, especially Te Reo Māori or a Pacific language, is highly regarded
You must also hold:
A full New Zealand driver’s licence and access to a vehicle
