PSW-GL assists in enabling individuals to continue living independently in a safe and healthy manner in their own bachelor apartment units, within the Gwen Lee Supportive Housing program location.
The PSW-SH works cooperatively with the Support Services Coordinator / Manager of Supportive Housing and Assisted Living in providing quality client-centered care, and in the effective execution of daily tasks. PSW-GL adhere to daily care schedules and overall care plans.
Gwen Lee Supportive Housing clients are typically able to direct their own care, are over the age of 60, and require the need for 24-hour assistance in daily living activities.
CORE DUTIES AND RESPONSIBILITIES
Assisting clients with daily personal care activities, in keeping with their individual care plan
Bathing
Dressing/undressing
Personal hair care
Shaving/grooming
Mouth care
Preventative skin care
Other personal hygiene tasks
Assisting clients with daily housekeeping activities, in keeping with their individual care plan
Assisting clients in daily activities which maintain adequate levels of physical health and safety, in keeping with their individual care plan
Performing safety wellness checks
Medication reminders
Living area visual scans
Engaging clients in positive/social conversations
Encouraging clients to direct and/or participate in their own care
Assisting clients with general mobility needs (walking, movement, positioning etc.)
Encouraging positive life skills
Assisting with basic range of motion exercises where required by the care plan
Demonstrating sensitivity and respect to clients who may be experiencing physical health challenges
Crisis intervention, in cooperation with or as directed by, the program Manager
Providing emotional and social support to both clients and their family members
Demonstrating a positive attitude and approach towards clients as unique individuals
Performs daily work in keeping with the established Gwen Lee Supportive Housing procedures, processes and expectations
Answers call bells in a safe and timely manner
Observes and records client information/comments/issues/behaviours/changes in the program Communication Book
Follows care plans as determined by the program Manager
Maintains a clean and organized work environment
Reports critical client care information/issues to Manager or Supervisor right away
Understands and follows individual client care plans, seeks clarification from Managers where care plan in unclear
Participating in team/group meetings
Other duties as requested or required

QUALIFICATIONS
PSW Certificate from an accredited training institution, required
1-3 years PSW experience, preferred
Member of the PSW Registry, preferred (may be required under some contracts)
Current First Aid and/or CPR Certificate, preferred
Training in gentle persuasive/non-violent crisis prevention, an asset
Previous experience working in a Long-Term Care or Supportive Housing environment, an asset
