Physical Therapist (PT) – Full-Time or Part-Time | SNF Hillcrest Health & Rehab – Bellevue, NE Innovate Rehab & Wellness Hillcrest Rehab Services is looking for a full-time or part-time Physical Therapist (PT) to join our Innovate Rehab & Wellness team at Hillcrest Health & Rehab in Bellevue! What You’ll Do As a PT, you’ll bring care plans to life using evidence-based techniques that improve function and quality of life. In this role, you will: • Deliver skilled therapy treatments aligned with established plans of care • Progress patients toward mobility and functional goals • Collaborate with therapists, nursing, and interdisciplinary teams to ensure seamless care • Assist with patient transitions across service lines • Accurately document treatments and complete timely billing/charging • Delegate tasks appropriately to rehab techs and support staff • Communicate effectively with physicians and care teams to optimize outcomes
What You’ll Need • Graduate of an accredited Physical Therapist program • Active PT license in Nebraska (or compact state) • BLS certification (AHA required) • 2+ years of experience in SNF, post-acute, hospital, or similar setting preferred
