The team
The Brokerage Team plays a key role in sourcing care services for individuals who have been assessed as having eligible needs. Acting as a central point of contact for social care placements in Suffolk, the team works across Adult and Community Services, maintaining close and regular collaboration with care providers, health and social care colleagues. A core focus of the team is to reduce delays by ensuring people receive timely access to the care and support they need.
The team is dedicated to promoting preventative outcomes, safeguarding vulnerable individuals, and ensuring that support is arranged as quickly and effectively as possible. This is delivered in line with key directorate operating models, including Supporting Lives, Connecting Communities, and Signs of Safety.
