POSITION SUMMARY:
Adhering to System Values, Service Excellence and Standards of Behavior, the SUD Grant and Outreach Specialist supports the implementation, coordination, and long-term sustainability of The Doorway Substance Use Disorder (SUD) treatment program at North Country Healthcare. This role ensures grant-funded activities are carried out in alignment with NH Doorway and SAMHSA requirements, while advancing timely access to outpatient SUD services for individuals in a rural community.
The Specialist works collaboratively across clinical operations, intake, marketing and communications, finance, community partners, and regional Doorway stakeholders to support grant compliance, project management, community outreach, and referral coordination. The position serves as a key liaison to strengthen access points, improve care flow, and enhance program visibility and effectiveness.
ESSENTIAL QUALIFICATIONS
Education:
- Bachelor's degree preferred OR 5 years’ experience in public health, health administration, social services, communications, or a related field.
Certification:
- All employees and contracted staff will have either successful completion of an American Heart Association* accredited course in Basic Life Support (BLS), or ACLS and PALS combined. (for clinical staff), Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis.

- Strong project management, organizational, and documentation skills.
- Excellent written and verbal communication skills.
- Ability to collaborate effectively across clinical, administrative, marketing, and community stakeholders.
- Understanding of substance use disorder treatment systems and rural healthcare challenges.
Work Experience:
- Experience working with The Doorway NH system or other SUD access initiatives.
- Experience with SAMHSA or other federal/state behavioral health grants preferred
- Knowledge of screening, referral coordination, and care navigation models.
- Experience collaborating with marketing or communications teams.
- Familiarity with quality improvement, data tracking, or performance measurement in healthcare settings.
- Experience supporting grant funded programs, healthcare operations, or community-based initiatives.
ESSENTIAL FUNCTIONS:
- Support operational implementation of NH Doorway–aligned, SAMHSA funded activities, ensuring consistency with approved scopes of work, timelines, and performance measures.
- Assist with grant compliance including documentation, data collection, and progress reporting through collaboration with the NCH Grant Coordinator.
- Track Doorway related deliverables such as screening and assessment volumes, referral coordination, appointment timeliness, and engagement outcomes.
- Serve as a project coordinator for Doorway related initiatives, developing work plans, timelines, and task tracking tools.
- Facilitate cross-departmental collaboration with clinical, intake, quality, marketing, and administrative teams.
- Monitor progress toward grant milestones, identify barriers impacting access or flow, and elevate issues with proposed solutions.
- Coordinate and implement outreach activities that strengthen Doorway access points, referral pathways, and community awareness.
- Build and maintain strong working relationships with regional Doorways, hospitals, emergency departments, primary care practices, behavioral health providers, law enforcement, community-based organizations, and State grant administrators.
- Participate in regional Doorway meetings, coalitions, and planning efforts as assigned.
- Collaborate closely with marketing and communications teams to develop Doorway aligned messaging and outreach materials that are clear, compliant, and accessible.
- Support development of impact stories, service summaries, and outreach content consistent with SAMHSA and NH Doorway communication standards.
- Assist in targeted outreach to referral sources and underserved populations in coordination with marketing strategies.
- Track data related to Doorway access, referrals, outreach activity, and service utilization.
- Assist NCH Grant Coordinator with preparation of SAMHSA progress reports, Doorway reporting, and internal leadership updates.
- Contribute to sustainability planning and continuous improvement for Doorway supported services.
- Performs additional duties as assigned.
