Silverado Creek Seniors Community is seeking a Care Manager (RN) to join our leadership team. This is a full-time management position responsible for coordinating and supervising the delivery of resident-centered care within a supportive living or long-term care environment.
The Care Manager provides clinical leadership, supports interdisciplinary collaboration, and ensures care delivery aligns with professional standards, organizational policies, and provincial regulations.
KEY ACTIVITIES Leadership & Team Coordination
Recruit, orient, supervise, and mentor nursing staff
Provide performance feedback, coaching, and support progressive discipline when required
Promote a respectful, professional, and collaborative work environment
Support staff in advocating for resident autonomy, dignity, and choice
Resident Care Oversight
Ensure completion of comprehensive assessments (e.g., MDS InterRAI) and individualized care plans
Monitor care delivery and evaluate resident outcomes
Lead or participate in case conferences, family meetings, and interdisciplinary rounds
Oversee admissions, discharges, and transitions
Clinical & Operational Management
Provide clinical oversight to ensure quality care and best practices
Lead reviews of incidents, including medication and non-medication events
Support management of complex resident care issues
Coordinate clinical programs (e.g., wound care, immunization clinics)
Quality Improvement & Compliance
Participate in audits and quality improvement initiatives (falls, wound care, etc.)
Ensure compliance with accreditation standards, provincial requirements, and Park Place policies
Review and submit clinical and operational reports
Participate in interdisciplinary committees and quality forums
Communication & Collaboration
Act as a liaison with residents, families, physicians, and external partners (e.g., Alberta Health Services)
Facilitate clear communication across care teams
Share updates, memos, and operational information with staff
Staff Management
Oversees recruitment and licensing compliance for LPNs and HCAs.
Manages attendance, vacation approvals, and WCB-related staffing issues.
Promotes staff morale and provides mentorship and coaching.
Coverage & Contingency Planning
Coordinates coverage for planned and unplanned absences.
Ensures clear communication of coverage plans to leadership, staff, and external partners.
HEALTH AND SAFETY RESPONSIBILITIES:
Responsible to read, understand and comply with Park Place and provincially mandated Occupational Health and Safety policies and safe work practices and ensures mandated employee access to Worksafe BC website or manuals.
Responsible to educate employees annually on ‘Violence in the Workplace” and “Bullying and Harrassment” in the workplace.
Demonstrates leadership through Involvement in all aspects of the Health and Safety Program including:
o Promotes, instructs, and monitors safe work procedures o Promotes health and safety policy and awareness o Implements effective safety suggestion o Ensures annual ‘Worker Survey on Violence & Aggression in the Workplace” is completed by employees o Reviews and posts OH&S minutes and ensure all “Actions’ are completed
Responsible to take every reasonable precaution to protect the safety of Residents, self, other employees, and the public.
Reports any near miss, injury, and accident or equipment damage to supervisor immediately and completes required reports.
Corrects and/or reports unsafe conditions.
Leads by example
QUALIFICATIONS AND EXPERIENCE:
Baccalaureate of Nursing, RN designation.
Minimum three (3) years of recent related experience in gerontology or long-term care.
Current practicing registration with the applicable provincial regulatory body.
Experience in leadership, team coordination, and quality improvement preferred.

Clinical Problem Solving: Ability to problem-solve clinical issues through knowledge and experience in gerontological, evidence-based practice and demonstrates ability to share job related information collegially.
Teamwork and Collaboration: Demonstrates ability to work together with others toward shared goals and desired outcomes.
Professionalism: Demonstrates ability to effectively establish professional working relationships with Residents, families, physicians, as well as both professional and non-professional members of the Care Team aiming for excellence in Residents’ care.
Change Management: Demonstrates ability to effectively introduce and manage purposeful change that is consistent with Park Place vision, mission, and strategic directions.
Communication: Demonstrates ability to communicate effectively with Residents, families, the public, medical staff, and other members of the interdisciplinary team. Ability to intervene effectively to resolve conflict and manage stress and work pressure.
Results Oriented: Ability to apply continuous quality improvement tools and processes within a dynamic environment, leading to improved care and service delivery.
Equipment: Demonstrates applicable computer skills and knowledge of nursing-related care equipment utilization and management.
WORKING CONDITIONS
Physical Demands – a mix of walking, standing, and sitting, with regular computer use and frequent movement throughout the facility.
Mental demands – the work requires extended periods of concentration, multi-tasking and the prioritization of work. The work is subject to the pressures created by constant interruptions, time constraints and multiple deadlines.
Working Environment – The work is carried out primarily in an office environment
