

As a Palliative Care Nurse Practitioner/Care Coordinator, you will be part of the largest multi-specialty care system in the Twin Cities. This position will provide fieldwork with seeing patients in their homes and facilities as well as telehealth with virtual video and phone visits. Local travel and HIPAA compliant remote work area required.
This individual will provide consultative health care for patients at home and in assigned care settings
Provide care coordination to achieve patient centered, high quality and cost-effective care across the continuum
Provide nursing leadership in defining and achieving program goals in a changing healthcare environment
Utilizes principals of quality of life, maintenance of optimal function and the patient’s advanced directives in developing plan of care

MN RN and APRN licensure required along with prescriptive authority
Hospice or Palliative Care experience (NP or RN) preferred
Nurse Practitioner experience in Hospice or Palliative Care is preferable
Must be able to provide own transportation for local travel

WORK SCHEDULE:
8am – 4:30pm: This is a part-time 0.40 FTE position; with expectation of working a hybrid practice of one virtual day per week and one in-person day per week
