

We are seeking a full-time Licensed Practical Nurse (LPN), Behavioral Health to join our Assertive Community Treatment (ACT) team and provide compassionate, community‑based care to adults with significant mental health and substance use needs.

As an LPN on our ACT Team, you will be an essential part of a multidisciplinary team providing intensive, wraparound behavioral health services. You’ll deliver medication support, clinical interventions, education, and crisis response that help individuals achieve stability, independence, and improved quality of life.
In this role, you will build meaningful relationships, help clients navigate real‑life challenges, and ensure continuity of care across community settings. You’ll work closely with psychiatrists, therapists, co‑occurring disorder specialists, and other mental health professionals to support truly holistic treatment.

This full-time LPN role follows a Monday through Friday schedule with rotating on‑call responsibilities. Travel within the community is required.

Competitive Salary: up to$65,000 per year
Hourly Range: up to $31.25 per hour (based on experience)
