Community Care Physicians – Community Care Weight Management and Metabolic Care
Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Community Care Weight Management and Metabolic Care, a specialized, multidisciplinary program focused on helping patients achieve sustainable weight loss and improved metabolic health.
Position Overview
As an Advanced Practitioner in the Weight Management and Metabolic Care program, you will provide evidence-based care for adult patients with obesity, Type 2 diabetes, and related metabolic conditions. You will work within a highly collaborative, multidisciplinary team—including physicians, dietitians, behavioral health specialists, and clinical pharmacists—to deliver personalized treatment plans that integrate lifestyle modification, medical therapy, and ongoing support. Responsibilities
- Conduct comprehensive evaluations for adult patients with obesity and metabolic conditions
- Develop and manage individualized treatment plans, including lifestyle interventions and medication management when appropriate
- Monitor patient progress through routine follow-ups, adjusting care plans to support long-term success
- Provide education on nutrition, physical activity, and behavior change strategies
- Collaborate closely with dietitians, behavioral health providers, pharmacists, and physicians to ensure coordinated, team-based care
- Order and interpret diagnostic studies and manage comorbid conditions such as diabetes, hypertension, and hyperlipidemia
- Maintain accurate, timely clinical documentation in accordance with organizational standards

Competitive Compensation starting at $125,000
Qualifications
- License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
- Physician Assistant:
- Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
- Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
- Current certification by National Commission on Certification of Physician Assistants.
- At least one year of experience as a physician assistant in ambulatory health care.
- Current CPR certification.
- Nurse Practitioner:
- Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
- Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
- Successful completion of pharmacology coursework acceptable to NYSED.
- Three years of professional nursing experience in an ambulatory setting preferred.
- At least one year of experience as a nurse practitioner in ambulatory health care.
- Current CPR certification.
