Licensed Practical Nurse, Pediatrics-Plaza Court

The Licensed Practical Nurse (LPN) provides care to designated patients within the Pennsylvania Licensed Practical Nurse Scope of Practice. Provides care coordination across the continuum. Participates in Network quality improvement initiatives. Demonstrates annual competence with clinical skills, clinical equipment, and electronic medical record system. Performs administrative functions as necessary.
Education:
- Graduate of an accredited Licensed Practical Nursing Program
- Bachelor’s degree in a health science, or a related field, preferred.
Experience:
- 1 – 2 years of experience working in an office based medical clinic, preferred.
Licensure:
- LPN license in Pennsylvania
- Basic Life Support (BLS)

Functions as part of a practice care team to implement and evaluate the care and education for designated group of patients.

- Adheres to the VHPCHC Confidentiality policy without exception and consistently takes measures to protect patients Protected Health Information (PHI) per HIPAA guidelines.
- Demonstrates knowledge of & applies patient centered care concepts and respects the rights of patients & their families.
- Identify patients with actual or potential health care needs that would require care. Follows established workflows to close care gaps.
Maintains clinical competence and demonstrates proficiency with the Organization’s quality improvement initiatives.

Performs selected administrative functions to support patient care delivery and administrative office responsibilities.
- Demonstrates appropriate non-verbal behaviors.
- Facilitates coordination of care across settings for a designated population, assuring that labs, studies, and referrals are completed and results discussed with the patient. Emphasis on seamless transitions in and out of inpatient settings, including establishing direct linkages to inpatient care teams.
- Maintains knowledge of registration, insurance, managed care plans, medical authorization requirements, and referral coordination.
- Performs patient care and procedures, utilizing sound clinical judgment, approved procedures/techniques, equipment, and supplies. Adheres to VHPCHC policies, procedures, standards, and guidelines.
- Provides phone advice to patients and responds to patient inquiries using sound clinical judgment, protocols, and established VHPCHC guidelines.
Provides coordination of care across the continuum (inpatient/outpatient/community) to assure appropriate utilization of clinical and community resources.
- Coordinates internal and external appointments for new and established patients based on patient’s medical need, preference, and the practice schedule.
- Functions within the Scope of Practice of a Licensed Practical Nurse. Refers problems beyond the Scope of Practice appropriately.
- Intervenes to prevent, minimize, or correct actual or potential patient risks based upon observation of physiological, behavioral, and environmental factors.
- Provides education to patients, assures education materials are current & appropriate, and evaluates the effectiveness of patient education. Refers all questions outside the scope of responsibility to the appropriate individual.
