Essential responsibilities include but are not limited to:
Perform administrative and resident care duties as assigned
Administer medications, treatments, and prescribed care; record vital signs
Maintain up-to-date knowledge of resident conditions and communicate effectively with the care team
Give and receive shift reports; handle telephone inquiries
Transcribe physician orders accurately
Assess resident needs and report changes in condition
Maintain accurate charting and medication records
Qualifications
Graduate of an accredited school of nursing
Active Licensed Practical Nurse (LPN) license in New Jersey
CPR/AED certification required
Experience in hospital, long-term care, subacute, or rehabilitation settings preferred
Experience working with geriatric populations is a plus
