Due to continued growth, we’re looking for a Care Manager / Care Partner to join our team.
The Role
In this role, you’ll provide person-centred care management to older Australians receiving support at home. You’ll be a trusted partner for clients and families, helping them navigate their care journey while supporting independence, wellbeing and dignity.

This role involves travel across the East, South-East and Inner Metro regions, with occasional travel to the North-West (travel reimbursed).
