The Alumni & Continuing Care Coach is responsible for guiding clients through their transition out of Onsite programs, helping them build and sustain meaningful aftercare plans while maintaining strong relationships with referral partners.
This role is where transformation is protected and extended. Healing doesn’t end when a client leaves a program—it evolves in the real world. You will walk alongside clients in that critical re-entry phase, helping them stay connected, supported, and grounded in their progress. Your work ensures that clients don’t just experience breakthrough—but continue living it, with the right resources, relationships, and encouragement surrounding them.
