Lead and facilitate detailed Root Cause Analysis for critical equipment failures and safety incidents.
Use structured RCA methodologies (TapRooT, ICAM, 5‑Why, FTA) to identify technical, human, and systemic root causes.
Develop recommendations and corrective actions with responsible departments.
Maintain the RCA database, track implementation, and verify effectiveness of actions.
Conduct post‑implementation reviews and monitor recurring failure indicators.
Train internal teams on RCA techniques and investigative tools.
Support Management of Change (MOC) reviews related to failure mitigation.
Competencies:
Strong expertise in RCA tools (TapRooT, Apollo, etc.).
Analytical thinking with ability to identify systemic causes.
Excellent facilitation, interviewing, and communication skills.
Technical understanding of equipment failure modes.
Ability to evaluate evidence, timelines, logs, and technical documentation.
Strong report writing and investigation documentation skills.
Certified TapRooT® Investigator (Essential)
Apollo Root Cause Methodology certification (optional)

Education:
Bachelor’s degree in Engineering (BE) or an equivalent qualification from a reputable university.
Preferably Mechanical, Electrical or Instrumentation
Experience:
Min. 15 years relevant technical experience in engineering field in large Refinery or Petrochemical
Min. 5 years experience in RCA facilitation and defect elimination process is a must.
