Community Navigator

ADA Australia is currently seeking a Community Navigator to join our ADA Link team. This role is ideal for someone who shares our commitment to social justice and is passionate about supporting seniors to navigate aged care, health and community systems with confidence.

About the role

In this rewarding community-based role, you will provide information, guidance and practical navigation support to seniors, helping them understand and access the services and supports they need to live well and stay connected.

You will work proactively with individuals, carers and community networks to reduce barriers to access, build trusted relationships and improve service connections through assertive outreach, collaboration and follow up support.

A key focus of the role is building strong local partnerships and maintaining up to date knowledge of relevant systems, legislation and pathways to ensure high quality, person centred support.

This is a work from home and community based role, requiring you to work independently while engaging face to face in community. This role may be best suited to someone who lives on the Southern Moreton Bay Islands (Karragarra, Lamb, Macleay or Russell Islands) or within easy reach of ferry services. Some local travel between the Southern Moreton Bay Islands is required, along with occasional travel to attend training, development activities and events. Approved work related travel will be undertaken using your own vehicle, with kilometre allowance paid for approved travel.

Key responsibilities

• Deliver high quality information, guidance and navigation assistance to seniors seeking access to services and programs.
• Provide individualised support to help seniors understand and access aged care, health, social and community supports.
• Assist clients to engage with My Aged Care and other referral or assessment pathways.
• Conduct regular follow ups to ensure ongoing connection with services.
• Undertake assertive outreach to identify and engage seniors, carers and individuals who may be isolated or vulnerable.
• Build and maintain strong relationships with service providers, intermediaries, community groups and networks.
• Participate in community engagement activities, forums, workshops and events.
• Maintain accurate client records, case notes and reporting.
• Identify service gaps and contribute to continuous improvement initiatives.

About you

• Tertiary qualification in human services, social work, behavioural science or community services or an equivalent combination of relevant experience, education and training.
• Demonstrated experience working with older people, vulnerable populations or people experiencing barriers to accessing services.
• Knowledge of aged care, health and community support systems, or the ability to quickly develop this knowledge.
• Strong verbal and written communication skills.
• Sound administrative skills and confidence using Microsoft Office and client information systems.
• A strong commitment to person centred practice, empowerment and continuous quality improvement.

Essential requirements

• Valid Australian Driver’s licence.
• Current (less than three (3) years old) National Criminal History Certificate.
• Valid NDIS Worker Screening Clearance.
• Valid Working with Children/Blue Card.
• Access to your own roadworthy, comprehensively insured vehicle for work related travel (kilometre allowance paid).
• Willingness to undertake some travel between the Southern Moreton Bay Islands, attend training or events as required.
