ADA Australia is currently seeking a Community Navigator to join our ADA Link team. This role is ideal for someone who shares our commitment to social justice and is passionate about supporting seniors to navigate aged care, health and community systems with confidence.
About the role
In this rewarding community-based role, you will provide information, guidance and practical navigation support to seniors, helping them understand and access the services and supports they need to live well and stay connected.
You will work proactively with individuals, carers and community networks to reduce barriers to access, build trusted relationships and improve service connections through assertive outreach, collaboration and follow up support.
A key focus of the role is building strong local partnerships and maintaining up to date knowledge of relevant systems, legislation and pathways to ensure high quality, person centred support.
This is a work from home and community based role, requiring you to work independently while engaging face to face in community. This role may be best suited to someone who lives on the Southern Moreton Bay Islands (Karragarra, Lamb, Macleay or Russell Islands) or within easy reach of ferry services. Some local travel between the Southern Moreton Bay Islands is required, along with occasional travel to attend training, development activities and events. Approved work related travel will be undertaken using your own vehicle, with kilometre allowance paid for approved travel.
